Retreat!!!!!

Ahh! I finally have a bit of time to stretch my legs, kick back, and relax... It's the Clerks' Retreat ladies and gentlemen, 3 days of solitude with no patients and no cares in the world whatsoever. :D

My bags are packed and I'm here at the LRU just chillin' while waiting for the people to get together. Man, this feels great! I know I left quite a big gap between entries and I haven't really been up-to-date with my stories per rotation, but I don't feel like doing that right now. Maybe later when I've moved on to the electives. =D

But for now, pahinga muna tayo... See you in 3 days... =)

Stallions MD: Congratulations!!!!!!

Taken from a post at the Xavier School website (www.xs.edu.ph)

Two Xaverians Top Medical Board Exam

August 2006 Boards Produces Ten New Doctors

Cesareo U. Tongco II (XS ’97)

The Physician Licensure Examination in August 2006 was topped by two Xaverians. Patrick Leonard G. Co (XS ’96) bested 2,696 examinees from all over the country to emerge as the board topnotcher, #1 overall. Nathaniel L. Yu Chua (XS ’97) also made it to the top ten, he tied for #8 overall. This is probably the first time that a Xaverian has become the #1 board topnotcher, and certainly the first time that two of our own have made it into the top ten.

Eight other Xaverians were among the 1,491 new doctors. These are Ishmael Christian D. Ching (XS ’97), Vicente Francisco Q. Firmalo (XS ’97), Patrick H. How (XS ’96), Rey Emmanuel S. Lu (XS ’97), Fernando A. Melendres Jr. (XS ’97), John Eric N. Tan (XS ’97), Kimberson C. Tanco (XS ’98), and Cesareo Amadeo U. Tongco II (XS ’97). The Oathtaking of the New Physicians was held on Sunday, September 17, 2006 at the PICC Plenary Hall.

Both topnotchers, Patrick Co and Nathaniel Yu Chua, were from UST, as was Kimberson Tanco. Ishmael Ching, Vicente Firmalo, Patrick How, Rey Lu, and Cesareo Tongco II, took Medicine in UERM-MMC. John Eric Tan was from De La Salle - DasmariƱas, while Fernando Melendres studied at UP-PGH.

Legal Med, Ophtha, ENT, and now Derma...

Well, that was quite a month that passed by...

I've been through 3 rotations since the last entry. And I've yet to continue work on my much-heralded San Lazaro Chronicles, which is merely gathering dust here at the DRAFTS section of my blog..

But come to think of it, it dosen't seem as important as my Fabella stories 'coz of the simple reason that we were not the first of our batch to rotate there (at San Lazaro). So I guess I might have to put it off indefinitely.. The hoispital stay there was unremarkable anyway. Hehehe

So off we went to Legal Med, or what they called The Medico-Legal Department in the hospital. I won't go through the day-by-day occurences because much of it was pretty dull anyway. Duties were still every 3 days, but the pre- and from-duty posts really rocked. =)

PRE-DUTY:
You're supposed to report to the PNP Crime Laboratory there at Camp Crame. There you would encounter rape cases, where you're supposed to examine the victim for documentation. Eh siyempre since I'm a guy, my job was purely clerical.. which suits me just fine... =D There were also some cases of physical examination of prisoners prior to transfer, which seemed like a formality more than anything.

DUTY:
The most toxic of the posts. You stay in UST for a good 24 hours (yes, its true, only 24 freakin hours!!) to answer all calls from the ERCD or TR-Pay for medico-legal cases, which you have to document via history and PE, let the relative and attending physician sign, and go back to the legal med office to type the rest of the papers! Its that freakin' simple!! =P Well, it should be simple as long as there aren't TOO MANY cases/calls that come through... Let's just say that during my duty night (with Johnny O. as my ever trusty sidekick), we were called down for 12 cases, 5 of which were people involved in the same vehicular accident!..And that was at around 9pm up to about 2am!!! Enough said... But in fairness to the "most nontoxic rotation" label, it was only here where I was able to actually finish volume 1 of the Handbook of Must-Knows! I just wish we had a laptop during those boring duty nights. Nakakasawa nasi yung sounds sa PPC ko eh..

FROM-DUTY:
This one takes the cake... UST expects us to be at Crame.... Whilre Crame expects us to be at UST... Go figure.. ;D

After 8 wonderful days at Legal Med, it was time to move on to Ophthalmology

Ok, here goes...

Ophtha was a bit different as compared to the other services I've rotated to, since it was the first time we got to experience "duty" status that dosen't require you to stay for the full 24++ freakin' hours! :D Non-duty clerks had to man the OPD and face the seemingly never-ending masses of ophtha patients that come to consult every day. Duty days were great, since you go home by 7pm. Other than that, I found my stay in Ophtha less than appealing... In fairness to the department though, I was really able to learn a lot, especially with regards to the proper ophthalmological physical examination. I even had the chance to observe a phaecoemulsification with IOL placement done by Dr. Say himself!! Ang bilis! At ang galing!!! :D

But there's a flipside to every story... Lets just say that in my opinion, the supposed "niceness" of the ophtha staff is seriously overrated.. and I'll just leave it at that... In other words, I left the service with a bad taste in the mouth..

After another 8 days, it was time to go to the department of Otorhinolaryngology (aka ENT)...

With ENT, the group prepared for the worst.. Feedback from other groups had made ourselves brace for the worst posible scenarios.. Impressions at the time were the following...

1. ENT has EXTEMELY LONG ORs (20 hours long pa daw, or even 8 hours just for a simple thyroidectomy)
2. ENT residents are really strict and mean
3. ENT duties are the most "wasakan" duties this side of Medicine
4. ENT leks to mass-admit on weekends

I won't argue with #3 and #4, since the duty days were REALLY TOXIC, and they really are fond of mass-admitting patients on Saturdays and Sundays, but I feel that they've been given a bum rap with regards to #'s 1 and 2. I myself assisted an ENT total thyroidectomy, which took about 4 hours. Not really ala-GenSurg speed, but reasonable enough, since they (ENT) take great pains not to hit the recurrent laryngeal nerve and preserve vocal function. And the ENT residents really rocked! I mean, they may be strict at first, but if you get to know then, they;'re actually a pretty cool crew. :D

Yes, contrary to popular belief, the ENT people do have ORs. That's why the department is called ENT-HNS (HNS stands for Head and Neck Surgery)

Here's a rundown of the duty posts:

Pre- and From-duty:
Similar to Ophtha, non-duty clerks have to go down to man the OPD at the ground floor. There you perform the complete ENT physical examination, using such tools as the ever-popular otoscope to the lesser-known items like the nasal speculum and the laryngeal mirror (which is more familiar in the hands of dentists). We encountered the more common cases that you see if you do OPD work at ENT like impacted cerumen, acute and chronic otitis media, pharyngolaryngeal reflux, postnasal drip, and even some cases of goiter presenting with an enlarging neck mass (duh!) with/without hoarseness.

I really enjoyed the OPD time at ENT. There's just so much you can do! The residents realized my enthusiasm and gave me the go-signal to perform some simple procedures done at the OPD (which the patients have to pay a little bit extra to have done :D) like ear irrigation to remove impacted cerumen. It's just too bad they didn't let me use the vaccum yet.. hehehe

I'm really giving ENT a serious thought when the time comes to choose Elective rotations. :D

Duty:
Duties, on the other hand, were just plain terrible! In the words of our interns, it was ":THE WORST". I mean, you're just a team of 2, running the entire 2 wards (which were filled with patients over the weekend) with various "weird" tasks like CT scans, neck ultrasounds, Thyroid function tests, Radio-onco referrerals, applications to St. Cosmas, clinical abstracts for DOTS treatment, and many many more...

Sobrang nakakapagod talaga... but in fairness to them, the staff aren't that strict when it comes to monitorings.. heeheehee.. plus the fact that you get another chance to "make money" from walk-in patients from the ER by way of the ever-famous "donation box" =D.

In short, ENT was very tiring, but it was a lot of fun. ORs I attended were a Total Thyroidectomy and a Tracheostomy (where thery even let me suture!)

That's enough for this entry. I'll follow-up later on with stories from Derma and (hopefully) the San Lazaro Chronicles..

Surgery Backtrack and a sneak peek at the San Lazaro Chronicles

On the prowl again here at the LRU.. Some things were never meant to change.. ;)

Just got back after duty at San Lazaro... you know, THE official infectious diseases hospital of the Philippines.. Good Lord, I just had duty at the dreaded Pav 10, the TB WARD... Tsk tsk... I can only shake my head at the state of things that I saw.. But I won't get ahead of myself and stick to the chronology =P

Surgery... What can I say? I'm really grateful that the teams agreed to slide for Neuro/Plastic and I'd like to congratulate our group for a job well done!

Shoutouts go to the Neurosugery team (Drs. Ranoa/Navarro/Pacia) and the Plasticman himself, Dr. Butch Mapayo! Astig kayo doc!

By the way, I forgot to mention the ORs I attended/observed at this service, so here's a brief list..

NEURO
Cranitomy (open up the skull to remove something from the brain)
Emergency Ventriculostomy (drill a hole through the skull to reach the ventricles and put a drain)

PLASTIC
Burn wound care (as in a VERY LARGE burn wound, encompassing the entire back, butt and some of the extremities)
Cheiloplasty (repair of cleft lip)

With Neuro-"toxic" behind us, we prepared to settle down to an easier time with Pedia/Uro, which was not really the case.. Urology is infamous for it's ultra-rapid turnover of patients, while Pedia referrals for Pedia Surg cases are at times toxic, to say the least...

Be that as it may, I really enjoyed this particular rotation, especially Uro (since my only patient at Pedia surgery was a case of a leg abscess which needed to be drained). The Uro ORs that I went to really rocked, although they looked pretty painful (well, from a guy's point of view anyway. I won't elaborate 'coz it might be too graphic...). I was able to observe a TURP and assist in an Anatrophic Nephrolithotomy and a Cystolitholapaxy.

Here are some short definitions to my dear uninitiated readers

TURP - short for Trans-Urethral Resection of the Prostate. Basically, you literally scape the enlarged protaste like wax from a candle though an acess port passing through the male urethra.. I won't explain how this is done.. You hafta research and imagine it nalang 'coz descibing it could make make some people flinch and my blog could lose what little readership it has. =P

Anatrophic Nephrolithotomy - you cut through the back to reach the kidney, cut open the kidney, and remove the stone trapped there. But believe me, its a lot harder than how I made it sound..

Cystolitholapaxy - another transurethral technique. You basically put a "nutcracker" into the urinary bladder by passing through the urethra, and use it to crush the stones trapped within. Then you aspirate the stone fragments via the same route.. Ouch...

It was times like these that made me grateful to the guy who invented anesthetic...

Thanks to the Urology team (Drs. Hipol III/Bisnar/Domingo/Ocampo) for making our stay as pleasant and interesting as possible, even though we screwed up big time with a particular patient. Thanks for not chewing us out even though you were really pissed off at what happened.

Also thanks to the Pedia surg people from GS (Drs. Dizon/Olalia/Alay-Ay/Nitisasmito/Manzo) during that time. You guys keep on rotating that it's hard to keep track with regards to who is assigned to what..

Moving on, we found ourselves at our last rotation at Surgery.. TCVS/Ortho, branded my some as the most benign rotation in all of Surgery. =D And it didn't disappoint one bit.. It was during this rotation that I was finally able to use the slippers I brought for exclusive use at the clerks room, and this was the time I got the most amount of sleep from any duty night I've been through so far... Not bad, not bad at all... =D

TCVS didn't have any ORs for me though. New TCVS patients are a rarity at the clinical division. But thanks to Dr. Jay Junio, the sole TCVS man of the hospital, we were able to observe at an Open-Heart Surgery there at the Pay division. He also let me assist during a bedside CTT insertion, and ako pa talaga yung nagpasok ng tube!!

*CTT - Chest Tube Thoracostomy. You cut a hole in between the ribs to drain whatever is in the cavity of the lungs which requires removal, be it extrapulmonary air, blood, or tissue fluid)

I also won't forget the times we went with him on rounds, and he gave me the power to write at the Physician's Order Sheets! Wow pare, parang intern! Astig!! =D It made me feel so powerful! Bwahaha! Wala lang...

For Orthopaedics naman.. I'm all praises for the Ortho team (Drs. Yumul/Tarigan/Angeles/Braganza). They made stuff seem light and interesting, especially Doc Braggy (who I happened to have already met a few years back when I was an MT intern and he was still in 3rd year med). Cool ka bro! Thanks for everything! I also had quite a bit of OR time at Ortho, assisting in an ORIF and arthotomy/synovectomy..

ORIF - short for Open Reduction Internal Fixation. Done for some types of fractures, it means opening up the patient, re-aligning the fractured bony parts, and applying internal fixation devices (like screws and plates) which are left inside the patient's body.

Arthrotomy/synovectomy - Literally means opening up the joint cavity/removing the lining of the joint cavity

Before we knew it, Surgery was over! And to think that at times it just seemed to drag on forever...

We held a sportsfest at the end of the rotation. Ang gagaling nung mga residents sa basketball! Tambak kami 30+ points!! =(

We also had a grand duty which I was unfortunately unable to attend.. Not that sobrang naghihinayang ako or anything, coz I heard that it was too loud for my taste anyway. =D

Alas, all good things must come to an end.. Id like to thank my surgery teammates for all the laughter we've shared and all that we've been through together, and I'd also like to say sorry for anything I've done (or not done for that matter)...

My Surgery groupmates...

Osang a.k.a. "partner"
Em
Rhyan
Leigh
Tricia
Marlon
Au

Thank you guys for everything!! I won't forget yung mga pinagsamahan natin!!

I'd also like to give a shout-out to my dutymates (the people from the other services na palagi naming ka-duty ni Osang, well, except for the time na nag-sliding kami.. (and nung nauso na rin ang sliding sa inyo) hehehe

Joanne/Mary/Maan
Braix/Dinah
Michelle/Ira/Jacky
Kathy/Aina/Grace
Lawrence/Christine

It was loads of fun despite the toxicity! I'm glad to have known some of you better during the bonding seesions. Thanks for the memories friends!!

Sliders

Yup, that's what we are... sliders..

In med talk, "sliding" is defined as going direct from one duty to the other (Duty-From-Duty-From)

And believe me, it hasn't been easy...

Our group decided to slide because of the apparent lack of manpower required when a group rotates to the Neurosurgery/Plastic surgery rotation...

And despite all the sliding, we can't help but feel that it still isn't enough..

But we get by, somehow... We're currently wrapping up the Nuero/Plastic rotation. Two more freakin' days to go. Then we "unslide" and return to the old pattern which recognizes a "pre-duty" status.

Last night has got to be one of the worst days to ever hit a neurosurgery duty team. Grabe, last night, we were undermanned, overworked, and underslept (if there was ever such a word.) as in WALANG TULUGAN!! The day started out neatly enough. Not many ORs since it was a Saturday, however, we had one in the PM when the non-duty people had already left. Not bad, I could handle the ward all by myself for the next couple of hours.. When my groupmates emerged from the OR, I felt such a great surge of relief.. WE only had one more OR to go later that evening which was merely a simple change of dressing for a severely burned electrical burn patient, hence the need to perform it in the OR. This poor guy had his skin burned off his entire back, parts of the back of his head, and even his ass!! Tsk! Tsk! And he already lost a leg during an earlier operation to save his life.

AFter that was over, we preppede ourselves for a relaxing night.. However, our peace and solitude was shattered by a call which told us to proceed immediately to the Emergency room of the Pay hospital (we were in the "clinical" division, a nicer word for "cheaper") to pick up a patient who was to undergo emergency crainiotomy ASAP! And there was another patient who needed to be admitted, but thankfully was in no need of an emergency operation.

Things got pretty wild back at the CD. Rhyan scrubbed in for the OR while Em acted as his runner, getting blood from the blood bank, etc. While little old me had to admit the additional patient and I was again master of the entire ward... plus the ICU, which got kinda spooky during the early morning because of the stories I've heard coupled with my wild imagination.. Lets just leave it at that for the meantime..

The fun never ends if you're in Neurosurgery... i hate it... bah! :P

By the way, belated Happy Birthday to my Mom who celebrated her 53rd birthday yesterday while I was at the hospital taking care of sick people... I love you mom.. :D

Nakakasawa...

Pre, Duty, From...

That pretty much sums up my life for the past 1-2 months...

And to tell you frankly, I'm getting sick of it..

Especially here in Surgery...

I mean, the residents are ok, the consultants are great, but I just can't take the srtess that come with everything, plus the fact that "from" status is not honored here.. which means that even though you've already finished a full 24-hours worth of duty (7am-7am), you're still obligated to stay 'til 5 pm!

If that dosen't suck, I don't know what does...

I'm presently pre-duty, but I still haven't shaken off the effects of the previous duty..

Oh yeah, and there's still an admitting conference later... bah....

Kung 'di lang sana talaga malaki yung ginastos namin pangmed.... hay....

Pagod, puyat, sakit...

I'm so damn tired...

I'm in the midst of our Surgery rotation, and it sure takes a lot out of you...

I thought the other groups/batches said that it would be benign?...

Anyway, that being said, the Surgery rotation covers a span of 2 months and is divided into 6 parts/services which you'll be rotating to: ER/OPD, GS 135, GS 24, Neuro/Plastic, Pedia/Uro, and TCVS/Ortho.

I won't be elaborating on the individual services for the meantime, since I'm just blogging on borrowed time here at the LRU.

Basta, nakakapagod talaga...

And to think that my cureent service isn't really one of the toxic ones...

Sigh... and the battle continues...

Back to the wards, my friend!

* I just discharged my patient today, a 68 y/o female with an admitting impression of acute ruptured appendicitis. We opened her up and found a gangrenous appendix. She was discharged improved and stable. yun lang po. =D

Gynecology, Obstetrics, and now Surgery...

Damn, time sure flies... (but it seems to drag on forever when you're toxic)

It's been more than a month an a half since my last blog entry...

I'm not sure what to write down, given that I'm always toxic and I don't have the luxury of time to write down everything that comes to mind..

Let me just say that I'm all praises for the Department of Obstetrics and Gynecology. They're very nice people, from the consultants to the residents, and even the current group of OB interns (who became close friends with our group).

Gynecology, our first rotation when we got back to UST, was kinda shaky for me. I really had a hard time adjusting to the more OC system in UST because it was SOOOO different from the workman's simplicity that we experienced in Fabella. On the plus side, it was there where I was able to handle my first patient, and I even got a free lunch for being a good boy. Hehehe.

But seriously, I was REALLY grateful that her family gave me the food when they did, 'coz it was pushing 1pm already and I haven't even had BREAKFAST yet.. I was damn near the state of total collapse when I recieved that gift (of food) which I will remember for the rest of my days.. Thank you Ma'am Agnes, you taught me a very valuable lesson in patient care and love for one's work...

Moving on to our 1-month stint with OB, it was the first time we were gonna be working with 8A, the other half of our clerkship group. It turned out to be a pleasant stay, as we worked pretty well as a unit (considering all the different hitches we encountered along the way). Too much happened in OB foe me to mention at this time, seeing as I have to prepare a report before I go on duty again tomorow. =( But let me just say that I'm also grateful to the OB-GYN consultant and resident staff foer making our 1 1/2 months stay there a very valuable learning experience. Despite all the toxicity and the poor accomodations (the OB Batcave), I really learned A LOT, expecially from OB.

Surgery was up next, we moved up last June 16, and we made the transition kinda smoothly I guess.. my present post is OPD/ER, which means that we man the ER on Duty days and take care of things at the OPD when we aren't (yes, even if you're FROM DUTY, kasi walang From status ang Surgery...).

I'll leave things at that for now. I have to get at least part of this stupid report done before I go to bed... sigh...
I guess that's enough

The Fabellista Chronicles: Days 9 to 15 (+1)

Day 9 - Hard Labor (PRE-DUTY)

LR duty is HARD WORK, no matter what people say.. Constant monitoring, extracting, IV insertion.. Its' just nuts man...

I don't really remember much from this day, proably 'coz it was too unremarkable. All I know is that through my experiences at the Labor Room, I finally learned how to insert a wide-bore IV line. :D

Days 10 and 11 - Running Man (DUTY and FROM)

Tried my hand at being the runner during dinnertime. Ang toxic pala talaga!! You have to order for everyone (including the residents, whom you have to take individual orders from), scrape the money together, follow up the food, and collect cash from the appropriate people. Man, it was even more draining than doing the normal runner jobs!! Since the group was kinda running low on funds, I decided that we should try something simpler and ordered our food from a nearby carinderia which luckily offered deliveries to the hospital. However, they didn't have drinks available, so I had the added job of "running" for drinks at the cooperative store located (thank God!) within the hospital premises. =D

The LR and DR duties were kinda uneventful, with only one sucessful delivery done and a solitary CS assist performed a little bit before going-home time. Added to this was the fact that the operation wasn't as simple as a CS, because the residents discovered an ovarian cyst which they had to remove, thus prolonging the operation and producing a histopath specimen that I had to worry about.. Good thing I was able to handle things quite smoothly amidst all the panic and despair the was beggining to envelop me.. 24 hrs duties can really do strange things to people.. Anyway, it was really taxing, and I recall thinking that I couldn't wait 'till the day I would walk away from there...

Day 12 - The Day Brian Turned Benign (WARD)

I really thought that this would be one of my best days at Fabella. My RIC and I sent most of our patients home the day before because she was having her oral examinations in the next couple of days. So she decided to take five and study for the exam the following day, which pretty much meant that I was free!! :D So I just went around during discharge time, helping my team otu do vitals on their patients. Things got a bit more complicated once we took our posts at the Maternal High-Risk Ward and the ICU, and guess who was stationed at the ICU?.. :P Anyway, things got pretty toxic 'coz I wound up with two patients there, one which had probable Takayasu's Arterities, and the other who was probably in sepsis already.. Damn, I was so angry 'coz I thought I already had it made for the day, but wound up going home 2 hours later 'coz we couldn't find the clerks we were going to endorese our patients to.. :(

Day 13 - Huling Pahinga (OPD)

OPD day was a blast! I only needed one more, and patients were aplenty. There were also student nurses there who took up most of the load, so we had lots of time to ourselves. :D Ahh.. I'll never forget these OPD days... :D

Day 14 - The Crazy Lady and Operations Galore (PRE-DUTY)

Started the day with only one thing on my mind.. I had to get my 2 remaining CS requirements before time ran out..

The start of the day proved to be quite amusing, though in a twisted kind of way.. there was this woman in the labor room who was having her seventh pregnacy, and she was wailing and whining like there was no tomorrow. I guess all the pain and the prolonged labor (I understand that she'd been there since the previous evening, I think..) Grabe, she was wailing even harder than most of the first-time pregnancies I've seen so far who normally would have a tougher time in labor and childbirth as compared to more "experienced" mothers.

To cut a long story short, she wound up making quite a scene outside at the corridors before eventually ending up undergoing a CS due to fetal distress.. how sad..

I got my wish for this day, and my CS requirements have now been completed.

Day 15 - One Last Push (DUTY)

My goal for this day proved to be a but trickier. I had to complete 5 NSDs within the day to be able to finish up my requirements. This hassn't been as easy for me as it seems, 'coz I still lacked confidence and experience in delivering, especially on my own. It didn't help that during those times when tons of primis seemed to come out of nowhere, I was either in the OR assisting in an operation, or doing my job as the designated runner... (to be continued...)

Day 15 + 1 - Fabellista Forever, but Fabellan no more.. (FROM/Start of UST Gyne Rotation)

The Fabellista Chronicles: Days 3 to 8

Day 3 - May hangin din pala sa Fabella (OPD)

My day pretty much started like the rest, with me running around using my XT talents to good use before attempting to do a smattering of progress notes.. sigh..

This has got to be one of the better posts for our stay at this hospital. At least you'll be free of the hot oven-like atmosphere within the hospital. OPD patients are seen at this covered open-air space located beside the hospital building. Ang sarap! You just sit around interviewing patients as they come before sending them into the OPD building. Chicken! :D And the best part of it is that you can complete a rotation requirement (10 History and PEs at the OPD) in one day!

However, things weren't as easy for me, 'coz I had to run back to the lab every so often to follow up the results of our patients... =(

Day 4 - Labor of Love (PRE-DUTY)

Things were gonna be different today, since it was gonna be the first time we would be in scrubs at our post in the LR-DR Complex (Labor Room, Delivery Room). Pre-duty clerks man the LR from 7-4, after which the Duty clerks (who are on DR detail) split up to cover both areas. LR jobs are quite simple and straightforward. You basically monitor the LR patients, insert IV lines, and get called to be the OR clerk when there's an operation to be done. Oh, and the most toxic job in the OR goes to the one assigned as the runner. Th runner is basically a gofer guy. He/she would not be in scrubs and had limited movement in the LR/DR complex. His/her job is to run to the lab and back with specimens, results, blood for transfusion, etc.

This day kinda went well. I wasn't a runner (thank God!) and I assisted in 2 cesarian sections(the requirement is 5).

Day 5 - Baby out!! (DUTY)

In any scenario, 24-hour duty is never a good thing. We braced ourselves for the worst.. and we got it.. In the delivery room, you are expected to deliver babies.. enough said.. However, we didn't know how yet, and thus we proceeded higher up the learning curve. Little did I know that at the end of the day (or should i say, at the start of the next), I'd find myself with skills that I never thought I had, doing things I never thought I could do...

The day went by slowly, with the residents patiently teaching us the intricacies of assisting normal spontaneous deliveries. We also had nursing students in the DR with us, as they also had requirements to complete. As a rule, all primigravids (that's first-time pregnancies to you non-med people) belong to us, the medical clerks, since they usually reqired episiotomy and episorraphy (in other words, incision and suturing). The multigravids (2nd pregnancy and up) came under the domain of the nursing people, since the vaginal outlet was usually already lax already after having previously given birth.

As the day dragged on, I found myself being amazed at the volume of women giving birth and passing out kids and placentas.. At 4PM, the LR guys left and our group was split into two to cover both terrritories. As a result, I attended two more caesarian operations, and tired myself out by being the runner from 9PM to 1AM..

Once I got back in my scrubs to man the DR, business was pretty slow.. until patients started coming in in packs until...

Day 6 - Placenta out!! (FROM)

I remeber it well.. it was around 1:30 am when I was able to complete my first episiorraphy (that's repair for you guys). I had to do it by hand, since I still didn't quite get the concept of instrument tying.. But a close wound is a closed wound, so chalk one up for the good guys. =D

When it hit around 4:00am, people were starting to get pretty tense and irritable due to the lack of sleep. But that's when the pregnant women started arriving, one after the other, and most of them were primis!!! =P Good Lord, I thought that day would never end!!!I was finally able to get the hang of instrument tying and performed my frist delivery. Cool pare. =D It was tiring, but well worth it.

At 8am, we were relieved by the incoming duty clerks and we proceeded to to our progress notes and labs before going home. I also prepared the laboratory requests that I was gonna use for the next day so I wouldn't waste too much time writing in the morning.

Up to this point, I still have no idea how I managed to stagger home after that very draining duty. What a night!!!

Day 7 - Discharges (WARD)

We had ward duty today. Fortunately, it was a Saturday, and we only had half-day duty! Unfortunately, I still had to run my usual gamut of laboratory examinations that needed to be done, plus two other patients who weren't under me but I still had to draw specimens from 'coz they were under the responsibility of the group. Once at the releasing ward, things went a bit more smoother this time around, seeing as we've already completed a full cycle of duties. At least we now had an idea of what to expect. Suddenly, the wards didn't seem as daunting as they were during our first day. The only downside to this day was that we were detained at the log-out point by a resident who was hell-bent on teaching us the finer points of proper endorsement.. Thank God Braix was there..

Day 8 - Fabella, benign?!? (OPD)

It's a Sunday, and the OPD is closed! Yahoo! This meant that all we had to do was finish up our progress notes for the day then we were home free!! =D I went along on my merry way, taking my sweet time 'coz I had all the time in the world to do the progress notes. Fortunately, Discharge day had just ended, so there were few patients left to work on, at least, until the next cycle of admissions and deliveries..

And so here I am blogging away 'coz we had it easy this time around. Tomorrow is Pre-duty day. I still need to get some more CS assists under my belt..

The Fabellista Chronicles: Days 0 to 2

Fabellista... that's a new term I made up for our group, the blessed clerks of our batch who get the privilege of plunging headlong into clerkship with Fabella as the first assignment. All the others who follow after us will just be called Fabellans. =P The OB orientation I had previously mentioned realized my deepest fears.. KAMI ang mauuna... sa Fabella!!! bwahahaha

For those who don't know, Fabella is an OB/Gyne and Pediatric hospital. In short, it's a freakin' HATCHERY of sorts. Or in the vernacular, PAANAKAN po siya na malapit dun sa Central Market. heehee

I never thought I'd see so many pregnant women in one place!!! Pucha, ANG DAMI!!!! Literally HUNDREDS of gravid women!

I'm a little behind in my blogging, so I'll have a bit of catching up to do, well I should at least complete this little project, the Fabellista Chronicles... =D

DAY 0 - Zero Hour
So named 'coz this was just the endorsement day. We weren't really expected at Fabella 'till the 16th but we went there anyway a day earlier to scout out the place and get a feel of the surroundings. And since the outgoing clerks weren't going to be there the day we went in, we wanted to get as much information as we could. There were 11 of us in the Fabellista group, or group 8b of the unitedclerks 2007, (Go united!!! =D) namely Jayveeh, Braix, Kathy, Mark, Ed, Binx, Pat, myself, Nats, Raissa, and Haids. I remeber the first thought that came to mind when I saw the hospital from afar.. "Oh my God, it looks like a freakin' PRISON!!!". Admittedly, the place has a kind of gloomy atmosphere that hung around it, but it wasn't what I was expecting.. pregnant women lying in the corridors giving birth out in the open, medical staff running around damp floors which are coated with God-knows-what, etc. wala namang ganun... Sobrang daming buntis lang nga. (as in!)

The outgoing clerks were kind enough to lend us some of their time and energy (tapos na rin naman kasi sila eh!) to orient us to how life is in the hospital. And we also had an orientation with the resident in charge of the clerkship training program there.

Just a short primer on Fabella life: The rotation is divided into 5; Duty, From, Ward, OPD, and Pre-Duty in that order. The only constants there are the toxicity due to the overwhelming volume of patients, and the neverending progress reports (more on this later).

After the orientation, the group trooped back to UST via pedicabs to have our little "refresher" orientation with the UST OB residents. Had lunch at the carpark KFC and saw the other groups there as well (it was Black Saturday by the way, which meant that the school was more or less deserted except for the hospital people). Grabe, lumalabas na ang pagka-clannish ng different (sub) groups. Almost everyone was eating with their own little "families". Grabe ang bonding, and clerkship hadn't even really started yet!

The OB refresher wasn't that interesting, so I'll just leave that out.

DAY 1 - The Beginning... (FROM) *Note, the words in all-caps within the parentheses are our group's assignments for that day

As luck of the draw would have it, me and Nats found ourselves with "From duty" status for the first day, which meant that we could just go for a courtesy call to our residents, but fate had other plans... After meeting with our Team leader, we were immediately assigned to do some progress notes!! Arrgh! Now, I would dare say that this is one of the most toxic things that we do in Fabella (other than helping the patients giving birth =P) We were a wreck! We only expected to be there for about an hour but would up gioing home a little before lunch! Braix wasn't available yet due to other commitments so we were pretty much on our own.. eh ang dami pa naman pasyente dun... and since the teams weren't complete yet, wala pang sistema yung ginagawa namin...

Went home tired and severly dehydrated.. (bought Gatorade at 7-11 before going home 'coz I was afraid I'd pass out while commuting) =c Halfway through the half-day, I was seriously comtemplating quitting and just going home... But still I kept on... hay.. I just hope one day I can look back at all of this and laugh.

DAY 2 - Andami naman nito!!! (WARD)

Today was an even greater challenge.. we were supposed to do progress on ALL the patients before 9am so that the residents could do a mass discharge. But at least this time our group was complete, so I feel that that went quite well.. although andami talagang pasyente. We were assinged to the different residents. I was paired up with Dra. Remo. Fortunately, despite all the toxicity, the Fabella OB residents are the kindest bunch I have ever met. They were really patient with us, understanding that it was our first ever rotation and stuff like that.

Found myself putting my MedTech skills to good use once again. It was kinda like I was on XT for the day, and I was running about the hospital like some crazy top. Man, I'm really glad that Fabella has only 3 floors... =P

We had a mortality today... sayang, 'coz that patient was already "to go home"...

It was kinda weird, watching someone that everyone was trying to resuscitate, seeing the life ebb from the person... (well, not really, 'coz I think the patient expired already when I got to the scene) I'll never forget this day.. ever... I mean, I've seen cadavers at Anatomy class, and even had the privilege of watching an autopsy during my MedTech internship, but this was very different...

To out things simply, I had another toxic day in one of the most toxic hospitals this side of the Pasig river... Now I have to sleep andsave my strength for the next day...

More to follow. Stay tuned...

And so it begins...

The LRU is open again.

And thus, here I am.

It's Clerkship Orientation day at UST, and I've just had a satisfying lunch at the carpark with the guys. Clerkship groupings and assignments were handed out earlier today during the general orientation, and here we are waiting for the OB-GYN Orientation which will be held at the Delivery Room later at 1pm.

Our first rotation says "Gynecology/Fabella".. Damn... Sa paanakan agad kami sasabak.. But at least it'll be better to have it done and over with early on rather than be toxic during the latter part.. (I hope..)

I left a pretty big gap between entries due to practical reasons. Internet access is an important commodity at home during these summer months (especially for my brother) and I've decided that my mundane blogging bisn't worth the additional expense. And besides, I'm usually tired at the end of the day, just when I've thought of something to write..

Last Thursday (it's currently Tuesday), I went to school early to enroll so that I could get home and change 'coz we were going to Tagaytay in the afternoon. There were 12 of us who went there: Me, Justin, Rhyan (yes, that's how his folks spelled his name), Alex Pisig, Al, Aldwin, Cocoy, Gina, and Cocoy's Angels (Nice, Ami, Cher, and Rochelle). The trip was basically uneventful, and the stay wasn't really that remarkable either. I find it too taxing to put every little detail here so I'll just give the ones that I consider my highlights (in chhronological order):

Saw a shooting star - Yup, I actually saw one while I was out with Justin looking for the girls who had inadvertentlu wandered off (the sudivision was kinda dark and their sudden disappearance left us kinda worried). Anyway, we both saw it and were giving each other high-fives and congratulating each other for witnessing such a rare event that we forgot that you were supposed to wish.. =P

Enjoyed the COOL night air - Brr! Tagaytay is COLD at night, even during summer.. but not like when we went there during sem break though.. now THAT was COLD!!!

Had dinner outdoors under the night sky - yeah, I know this may seem a little ordinary, but that day it was a lot of fun! Chef Peco made great sinigang and the Titan boys did a good job with the roasted chops. Yum!

Took a morning walk at sunrise - Now this felt great! I was justwalking around the subdivision at random, taking in the cool fresh air and relaxing my mind in general.. It was really a great getaway from the city heat and all the hustle and bustle of city life.

Had a fantastic game/practice - man, shooting the basketball never felt so good!!! In short, I think I was ON FIRE!!!! I was making jumper after jumper, even shot a buncha threes to cap things off (this was during practice ha? I settled for long twos during the game =D)

The road trip home - due to lack of transportation, the guys were forced to take the bus going home. We rode the bus from Tagaytay to Baclaran, and felt like we were trapped foreven in Cavite! Man, that province is HUGE!! Or so it seemed 'coz antagal namin dun. =P Finally, after getting to Baclaran, we rode the old LRT to UN Avenue. Good Lord! Ang INIT!!! =( After a brief refreshments break at a nearby McDonald's, we finally made it back to UST to try and get our classcards, pero wala pala eh.. =P I'm just glad I made it home in one piece without passing out from exhaustion..

Well, I guess that's it for now. Gotta go to the ward.

Clerkship?.. BRING IT ON!!!! =D

What a relief...

Went to school yesterday desperate for some news about the Medicine grades. It was a real nail-biter, as the Med department hasn't posted their list of "remediables" and "irremediables".

FYI:
Remediable - you failed the subject but got a grade of 70-74 and thus are eligible to take a remedial examination as a last-ditch effort to pass (removal exams sa ibang schools)
Irremediable - you failed the subject outright, garnerning a grade below 70. You're not qualified to take the remedials, and must take the subject again next year. =(

After a long and agonizing wait (with lotsa comedy and soul searching in between), Ma'am Jessica and Dra. Ruiz finally emerged from the Med office with the list of those who will take the remedials. (fortunately, nobody was irremediable in Med II. Go united2007!! =D) Lo and behold, MY NAME WAS NOT ON THAT LIST!!!

You might be asking, "what's the big deal? Eh di'ba may chance pa naman pag magremedials?" Yeah, that's true, but I don't wanna risk it. Going back to that dreadful time last year where I was faced with the giant evil monster called PHARMACOLOGY. I barely passed with the skin of my teeth. I mean, if you're gonna look at my average for the 4 quarters (which is just 80% of the grade), I just barely made it. But I failed in the grand finals (20% of the final grade).. BADLY, and thus I was on the firing line. Good thing they decided to curve and I got the +1 from the compre last year. I really dread taking the remedials 'coz I feel that I'd surely fail..

But all that is behind me now, and I can safely say that I an now a MEDICAL CLERK. =D

By the way, I got my +1 from this year's compre . =D May corrections kasi eh.. Either that or they only considered the part 1 of the compre (which I passed outright)

My prayers go out to my other batchmates who are taking their remedials next week. Good luck guys! Kaya niyo yan!

And my heart bleeds for those who didn't make it... Basta para sa amin, you're still a part of the batch no matter what!!

Long cold shower

*NOTE: Was supposed to post this yesterday but something went wrong with either Blogger or my computer, though I reall think it was the former... I'll post something current later on..

MARCH 27, 2006
6:50 PM

It's over...

Took the Compre today.. bagsak.. 88/140... needed 91 to pass (we use base 65 at UST Med) sigh... 3 freakin' points... =(

For the non-UST Med people, the Compre is basically a yearly examination which encompasses EVERYTHING you've taken up for the past year. And it wasn't easier for us incoming clerks, since they even threw us questions from 1st and second year!!! Not that I'm complaining of course.. Those basic questions pulled my score up somewhat, and besides, we should've know those things already!! =P

The Compre has no bearing on the grades whatsoever if you fail. However, those that pass the Compre (at least 65%), get a +1 to their final grade in EVERY subject. If you're really bionic and manage to score at least 85%, you get a big +2!!!

It's really freakin' hot here, one of the hottest summers I have ever experienced.. and with that in mind, I decided to take a long cold shower, one I haven't had in a LONG time. It was refreshing to just stand there and turn the heater off and let the cold water run down... No hurry to study or get dressed for school, no thoughts of the homework you have to do, no worries about any forthcoming reports or exams, nothing... My mind has never felt so empty or relaxed in such a long while... =)

Now what the hell do I do tomorrow? =P

Benign at last!!!

I'm celebrating my 1st day of freedom in a looooooong time.. =P Ang sarap!!!

That being the case, I thought I'd be a great time to indulge in a little solitary malling.. It may seem a little weird or lonely to you, but I really enjoy my solo flights. =) Nobody to pressure you, tell you where to go or what to do, no one to argue with your choice of food, etc.. I enjoy the total freedom of being able to do what I want, when I want, without any time pressure. =) Had lunch at the fastfood area where I ate Double porkchops at Sizzling Zone. Yum! =)

As expected, spent most of my time in the bookstores prowling around for a good buy. Eventually, I was able to find the second Republic Commando book! Astig! =D With morale running high after that prize catch, I swept the entire mall floor by floor. My bro texted me and reminded me of something else that I was planning to do at the mall, and that was to buy a pair of beach shorts for the C5ers outing on Tuesday!! Man, I had a really tough time deciding what to buy. Many thanks to the staff at Bench Shangri-La for patiently putting up with my fickle-mindedness. =)

At the end of the long day, I planned on grabbing cup of joe at either Figaro or Starbucks. But the places were packed, and I guess I'd seem kinda pathetic drinking my coffee alone in the midst of the crowd. Besides, someone had to go home and cook the rice before everyone got back.. ;)

And speaking of which, here they are! =P

A sigh of relief

I failed the Ophtha exam... fell 3 points short...

Good thing I passed in the final grade.. =) Whew, that's a load off my mind..

Medicine na lang...

Had lunch with Ed, Pat, & Boom at the nearby Jolibee.. What a way to celebrate huh? Jolibee.. sheesh...

Fortunately, Paolo texted Boom and asked him to join the C1 people to go watch a movie, and Boom was kind enough to ask if I could tag along.. =) There were 5 of us (Me, Boom, Paolo, Dana, and Michelle) and we rode in Dana's car.

So off we went to SM San Lazaro to watch a movie.. I didn't really care what movie it'd be (as long as it ain't horror) 'coz I haven't really been to the movies in quite a while. We kinda felt like caged animals that were just let loose. These past two weeks were just pure hell, and we DESPERATELY needed a bit of R&R. =P

It was my lucky day, as Dana (being the one who declared the movie trip) chose the movie I had been hoping to watch when I saw it in the papers, and it was GLORY ROAD.

It's a true story about the Texas El Paso Miners (NCAA Division I Basketball team) back in the 1950s. Man, racism was really rampant those days, especially in them southern states. =P Basically, it revolved around the efforts of their coach (forgot the name) to put good players on his team (regardless of race) and win. To cut the story short, they came in as a totally unknown team which blew everyone away and eventually faced the Pat Reily(yes, THE Pat Reily of the NBA)-led Kentucky Wildcats for the NCAA Championship.

It's an inspirational/feel-good movie for the basketball buff, but I guess it'd appeal to everyone as well. I give it a 5/5

Afterwards, we just kinda wandered around looking for a place to just sit back and chill out (preferably while munching something for merienda) until we eventually settled for GoNuts Donuts. Chatted there for some time while enjoying the great relief of having everything over with.

Went back to school with the guys while the girls went home. We found ourselves in front of the Ophtha board and that's where I saw that I had passed. =D

I say again, Medicine na lang...

Is it really over?....

I'm still in shock...

Just finished the Medicine grand finals... damn, they really saved the "best" for last...

120 items.. whew....

But it wasn't as hard as I feared it would be.. Looks like those guys in Medicine have hearts after all.. Or probably 'coz they don't want to wind up with an even smaller number of clerks come April... hehehe

I just hope I'll be one of those incoming clerks...

I still fear Medicine, given my lackluster performance during the shifting examinations, especially during the first shift... Damn, I really hope I'll make it..... Then I can take the game to a whole new level... I guess I'll just have to hope for the best and accept whatever God gives me...

I'm also a little worried about Ophtha... turns out I still needed 11 percent coming into the grand finals (20%) to pass..

It all seemed so ureal.. as I put my pencil down for the last time.. (like I said, I'm not really counting the compre) I stopped to savor the moment.. (hopefully, I passed so I won't have to go back on these statements) and I tried to look back at everything that I've been through these past 3 years.. but I thought better of it 'coz the prof might misinterpret my daydreaming as cheating. So I just passed my papers and here I am at the LRU. =)

I wanna go back to the beach...

The above pic came from the website of La Luz Resort there in Batangas.. went there with the family during the last sem break... ganda dun! White sands, clear water, corals, fishes, the works!!! Malayo lang nga... Check them out at www.laluzresort.com

Gotta go. I'll see if anyone wants to eat out.. =P

Friday, March 24, 2006

Isa na lang....

Naknamput@! Isang test na lang!!!!

Man, I am just so TIRED na... pero one last thing stands in my way...

I'm not counting the compre, 'coz it's basically a no-bearing exam, and besides, how the heck do you study for a 100-point exam which covers all the topics from all the subjects for the past year???? =P

Konti na lang...

Thursday, March 23, 2006

Tatlo na lang...

Whew! That went well...

Just finished the Surgery grand finals.. wasn't realily that hard.. may mahirap, may inulit, may kayang hulaan..
=P

Tapos I heard news daw na the Medicine finals is only worth 10%... sana totoo. =D

Man, there's really a dearth of people here at the LRU.. talagang sineseryoso yung finals ah. hehehe. Why am I here? Naghihintay ako ng samplex for Pedia. Eh since maaga ako natapos, I thought I'd do something cathartic while I wait for my "less-decisive" classmates finish their examinations. =D

It's about quarter to four.. I'd better get going.

Wednesday, March 22, 2006

Four down...

... and four to go...

We're currently in the midst of Grand Finals week, and all the medical students are having the time of their lives trying to cram an entire year's worth of readings into a single night.. night after night.... Go help us... =P

*I pity the freshmen.. this must be such a great shock to them... hehehe

So far, this is what I've been through...

Mon - Psychiatry
Tue - Gynecology, Legal Med/Med Juris, and Radiology

And these are the ones that lie ahead...

Wed - Surgery
Thu - Pediatrics, Ophthalmology
Fri - MEDICINE

I really dread Med.. (hey, that rhymes!!) Mahirap talaga kung tagilid ang grade, and the last exam is worth 20% of your grade for the entire freakin' year!!!!

The show must go on, and the fight continues... Aral lang ng aral!!!!

Saturday, March 11, 2006

I've got a bad feeling about this...

Was supposed to go apartment-hunting with a friend this afternoon after pedia surgery but thought better of it.. Call me weird, but I suddenly had this awful premonition that something bad is gonna happen if we went ahead with our plan...So here I am blogging away instead in the relative safety of the LRU.

I'm currently playing a Flash-driven online RPG called Adventure Quest. Simple gamelay with anime-ish graphics. Check it out here http://www.battleon.com

Darn, they're gonna close early today. Oh well...

Dakilang utusan: the future clerk

Even my brother likes the LRU.. and he's never even set foot in this place...

He sent me a buncha stuff that he got from the computers at school through email and wants me to download them for him.. so here I am... (grumble grumble... utos ng utos... grumble grumble..)

Broadband internet is so cool... :D

I'm contemplating renting an apartment for clerkship.. I hear that the schedule is gonna be a killer, and confounding that is the fact that we're gonna be a smaller batch of clerks compared to batches before..

More work for us... whoop-dee-do!.. :P

One word would be inadequate to sum up what I'm feeling right now as we draw ever closer to our next goal.. It's a whole buncha mixed emotions.. One one hand, I'm feeling relieved that I'm almost done with this part of my education, since it won't be as academic as before.. but then again, I'm apprehensive 'coz of the following:

1) I feel that I 'm inadequately prepared to be a clerk (just take a look at my grades)
2) the reported workload is a killer
3) there's a whole lot more responsibility involved (we're dealing with human lives here!!)
4) ... let's just say that I'm having second thoughts about certain people around me...

1-3 don't really bother me that much.. its the fourth item that could spoil everything.. Oh well, that's life I guess..

You really can't choose the people around you... I'll just try to make the most of everything.. Basta I'll do what's expected of me, but don't expect me to be amiable... I'm sick of people shutting me up whenever I have something to say or being ignored if I have an idea different from what the general public has.. (which more often than not turns out to be the right thing to do).. I'm really sick of being pushed aside... especially when I just act in good faith.. walang malisya.. nakakasawa na talaga eh... If you don't want me nice, then I hope you'll like me when I'm really nasty...

Good luck na lang sa clerkship

Saturday, March 4, 2006

Chill out..

Brr... it's cold...

The air conditioning here at the LRU is really doing its job.. But then again, this place is kinda empty at this time hence, less warm bodies that generate heat..

It's the weekend again!! Man, I'm so wasted... Must be all the fatigue of 3rd year creeping up on me...

Almost a clerk.. man I never thought I'd see the day... Grabe, it's been such a long and hard road... Sana makapasa...

Just got my lab results which are part of the pre-clerkship physical.. It seems like only yesterday when I first excused myself from the OPD lab to get my requirements together as an incoming freshman... Nakakaloko pala talaga when you look back where it all began...

Incredible... I still can't really believe that I've already made it this far...

Tomorrow, a new day begins....taking me one step further down the path called "Medicine"...

Being benign

I'm back in school.

We had the day off yesterday 'coz of some problem with national stability or something like that. An entire brigade of Marines holed up there in Fort Bonifacio were up in arms in support of their commandant who was apparently relieved for no good reason..

The political turmoil here in the Philippines is nearing its breaking point. I've tried to keep abreast with the news, but I guess too much is happening too fast, especially during Friday (?) where the country was put under a state of national emergency. This resulted in classes being suspended, but we still had a quiz in Medicine anyway before leaving for home.. Sabagay, 'tis a far better thing to have that exam when we did than to let it be set back until all the backlog would eventually catch up to us..

Is it just me, or is Pau Gasol looking more like a Grizzly every day?..

Guess he took his role to heart a little too much.. It must be the beard pare... hehehe...

Nakakagago... I'm not used to being this benign... Walang test mamaya.. walang test bukas... and baka pati sa Thursday wala din... hay... ang sarap! :D Maybe it's 'coz all the major paperworks are done, and we're just counting down the days 'til the start of clerkship...

I hope everyone will make it.. including me...

Xavier Captures Tiong Lian Titles
2/17/2006 10:17:47 AM
Henry Liao (XS '72)

Xavier School romped away with the championships in both the Aspirants and Juniors divisions of this year's Tiong Lian basketball competitions.

On Feb. 11, the Aspirants team defeated St. Stephen's High School in the one-game finals to finish with an unblemished 9-0 record and annex the title for the fourth year in a row.

Five days later (Feb. 16), it was the turn of the Juniors squad to win the championship in its division after whipping Chiang Kai Shek, 84-73, in Game Two of their best-of-three titular series for a 2-0finals sweep and a 9-0 finish overall.

The game was closer than the final score indicated. The Stallions, who secured a 72-61 decision in Game One, were tested to the limit from start to finish. CKS was ahead 70-65 with six minutes left in GameTwo. During the time, Xavier's 6-3 MVP Woody Co was on the bench for abreather and so was CKS's burly 6-6 center Justin Chua, who was temporarily sidelined by a foot injury. After the two big men checked in together at the four-minute mark, Xavier turned on the heat and CKS suffered through a meltdown.

Xavier outscored CKS 19-3 the rest of the way, including the game's final 10 points. Charles Tiu split his two free throws and knocked in a three-pointer to put the Stallions within striking distance, 69-70. A basket by Frederick Tan pushed CKS's lead to three, 69-72, at the 2:43 mark. A 5-0 run, anchored on Marc Tio's basket (71-72) and another three-point bomb by Tiu, gave the lead back to Xavier, 74-72,with 1:50 left. It was Xavier's first lead since 61-60 early in thefourth quarter. Following CKS's 6-4 Alexander Wong's 1-for-2 effort from the foul line, 74-73, and time down to 1:30, Co ignited a 10-0 that finished off CKS.

A cool and calculating Co connected on a three-point baseline jumperat the 1:08 mark, 77-73, sending the Xavier rooters in the capacity crowd at the UNO gym into a frenzy and confirming Co's status as the league's best player this season.

Thereafter, a stunned CKS squad lost its composure and surrendered meekly. Tio, who got two quick fouls that helped bring Xavier to penalty situation just four minutes into the first quarter, made2-of-2 from the charity stripe, 79-73, time down to 49 seconds.Subsequently, Irvin Sta. Maria put in a fielder, Tiu got a pair offree throws (83-73 that gave Xavier its first double-digit lead throughout the game) and Sta. Maria finalized the count by going1-for-2 from the foul line.

During the 19-3 span, Tiu totaled nine points, Tio had four and Sta.Maria and Co each contributed three.

Chiang Kai Shek got off to a a quick start, racing to a 5-0 lead before Co broke the ice for Xavier with a basket. The Stallions moved ahead after erasing a 10-8 deficit with an 8-0 blast, 16-8. FollowingTiu's second consecutive three-pointer that brought the score to19-12, in Xavier's favor, CKS also had an 8-0 run of its own to regainthe lead, 20-19. The first quarter ended with CKS ahead 22-20. Co scored Xavier's first 13 points and Tiu got his team's last seven inthe quarter.

Xavier trailed by as many as eight points (22-30, 35-43) in the second quarter and CKS owned a 45-38 advantage at halftime despite Co's 19 points, eight rebounds and four blocks.

The Stallions still trailed by eight, 46-54, midway through the third quarter before they clustered five straight markers (P Yao's three,Tiu's free throw and Torres' go-ahead two) to take the lead, 59-57,for the first time since late in the first quarter. The third canto ended in Xavier's favor, 61-58, on Jert Yao's inside twinner.

CKS opened the fourth quarter with six quick points to go ahead once more, 64-61, and was still led 69-63 at the seven-minute mark. Then Xavier made its move, a fierce rally that sent a number of CKS players into tears at game's end.

Charles Tiu, who was a member of the 2003 Aspirants championship unit(when his elder brother Chis Tiu was starring for the Juniors championship team that same year), had a career-defining game, at least at the high school level, in Game Two. Charles registered a career-high 30 points, 15 in each half. He pumped in five three-pointers and was 11-of-15 from the foul line.

Co, bucking a mild sprain, finished with 29 points (8-of-11 from the foul line), 13 rebounds and four blocked shots. Only six other guys scored – Philmon Yao, 7; Sta. Maria, 6; Tio and Phillip ConstantineOng, four each; and Alejandro Torres and Jert Yao, two apiece.

The Stallions were 26-of-34 overall from the free-throw line and totaled eight three-pointers (Tiu 5, Co 1, P Yao 1 and Sta. Maria 1). In contrast, Chiang Kai Shek was 19-of-24 from the charity stripe and only had a pair of trifectas.

Justin Chua led CKS with 24 markers along with nine boards and two blocks. Wong (nine rebounds) and Johar King added 17 points each in a losing cause.

For only the third time in Tiong Lian history, one school (Xavier) took home the Aspirants and Juniors division crowns in the same season. Chiang Kai Shek accomplished the feat in 1991 and Xavier turned in the trick in 2003.

Co scored at least 10 points in all nine games for Xavier this season, his lowest being 11 against Hope Christian and his highest being a tournament-best 53 against UNO. He reached the 20-point plateau six times and averaged 24.9 points per game.

Tiu averaged 14.2 points an outing while Philmon Yao hit at a9.1-point clip. Ong averaged 8.4 ppg and Tio normed 7.7 ppg for the season.

Nakakatamad.. =P

Another long week has come and gone.. sigh.. each passing day a reminder that we are fast approaching the next step in our medical evolution. CLERKSHIP... (I sure hope I'll be able to pass everything though..)

It's been a very slow week (relatively). No major quizzes or reports, no paperworks to pass, no nothing.. This week has just been an excellent time to chill out, which is what I'm doing now.. :D Hay, and sarap!! 8D

Don't get me wrong, things are starting to pile up for next week. But hey, I'm calm, cool, and in control.. the worst is behind me... and I don't think that things could any worse than it was.. (see previous post)

The five-foul throwout and the looming doom

It's Monday again... and I think I've just been through the most toxic week of my young medical life... :) Just look at this list...

Monday (1/30/06)
  • Medicine Quiz -> this went surprisingly well.. despite the fact na I haven't studied at all, I still managed to get through unscathed with an 8/10. ;)
  • Case presentation in Surgery -> Helped out the main reporter 'coz she was toxic enough as it is -> thus, i spent the last few hours of the weekend doing this
  • Psychiatry Quiz -> 10 points lang, but still, delikado pa din kami sa Psych, so aral pa din diba
Tuesday (1/31/06)
  • Pedia FCP Quiz -> Tried studying for this, but never got beyond barely passing. The Med and Surgery reports were already taking too much out of me.
  • Optha Quiz -> Dr. Sua decided to spring a nasty 5 point surprise on us. I only got 3...
Wednesday (2/1/06)
  • Gynecology Quiz -> went to school early to study for an 11am exam. Ok lang. Wasn't that hard. Sulit yung inaral ko. 'Di kasi ganun kahaba eh..
  • Surgery Journal Report -> At last, nakapagreport na din! I spent about 3 sleepless nights working on this.. Now I won't have to worry about this for the rest of the year!!! However, may kasunod na...
  • Surgery Quiz -> It was truly a week of surprises. We thought we got it straight na hindi kami magqquiz on this day, that we appealed to have it moved the following week. Guess not...

Thursday (2/2/06)
  • Pediatrics Quiz -> Regular stuff, but I wasn't able to study for this either 'coz I was too busy with the BGD report.. Good thing I passed... barely...
  • *Even though yun lang yung test, we were still frantically running around after class trying to get the BGD done.. At this point, I already had a bad feeling about this...

Friday (2/3/06) <- The mother of all toxicities... Eto yung araw ng 5-foul throwout
  • 1) Medicine Quiz -> I really lost out on this one. Was too tired to study, heck, I was too tired to think!! got 3/10...
  • 2) Medicine BGD -> this one takes the cake. It was a total washout!!! :( Looks like we spent too much of an effeort trying to get too much done for something that didn't really need that much effort. Spent a greater part of the week preparing for this. I was really draining, both physically and emotionally. Good thing I had my DWTL songs in the PPC to keep me sane through the day..
  • 3) Psychiatry Shifting Exam -> All the fatigue due to all the work and sleepless nights was beginning to take its toll. Good thing I was able to pull something out of my hat and answer this test intelligently.. I just hope the outcome is as good as my prognostication..
  • 4) Pedia Surgery Quiz -> Dr. Cua also decided to pull a fast one on us and gave us a 20-point quiz. But at least he gave us the rest of the period off to study for...
  • 5) Neurosurgery Shifting Exam -> I have absolutely no idea how I managed to get through this without having a really hard time.. I mean, I just studied about an hour before the test!! Credit goes to the "tanungan session" I had with the guys outside the room while waiting for section D (?) to finish their class. Sobrang high-yield!! Thanks guys!!
  • *They say sa baseball na "Three Strikes and you're OUT". Eh para sa akin, basketball ang analogy ko.. pag five fouls ka, ejected ka na (sa amateur) =P
* FYI, puyat ako the entire week, sleeping at about 12:30am or later every day and having to get up at 5am to go back to school for 7am classes (except Wednesday, wherein 11am first class namin. Pero nagising naman ako ng 7am to go to school). "Why was I always up?" you might ask.. Eh pano ba naman if you have to balance studying for quizzes, major exams, and big-time presentations spread evenly throughout the week, how the hell would you expect to get through all of that 'diba?

At least the worst is over... I hope.... Another BGD looms over the horizon, and the prognosis ain't looking any better due to some... intrinsic problems... hay.. the fun never ends...

At least medyo benign naman this week.. but then again, benignity is merely relative....

Hmm, come to think of it, pwede din na yung 5 days of the week yung 5 fouls ah.. =P

Legally Medicine...

Been hanging around here at the LRU since about 9:45 after finishing the Legal Med prelims examination. Wasn't as hard as I expected, but kinda challenging din naman, especially since 'di naman ako nakapagaral ng mabuti.. =P It was a short exam, only 50 items!! I just hope my luck held up today as I guessed my way through roughly half of the test...

I also took another test of sorts when I got here.. It's a kind of aptitude test which matches your "attitude" to particular fields of specialization. You can take the test at this site: http://www.med-ed.virginia.edu/specialties/

The results showed that the number #1 speciality match for me was Endocrinology (ugh), followed closely by Derma (Santa Banana!)

Longtime favorites Neuro, Patho, and Gastro were placed at 5, 6, and 17 respectively...

This got me thinking.. "Accurate ba talaga 'to? I mean, Internal Med and Neurology have always been frontrunners on my list, but Derma?... Bagay ba ang ugali ko sa Derma?... Hmm, kung sabagay, pogi naman ako.. bwahahaha!! " Joke lang! =P

Below is the complete list and rankings:

1 - endocrinology
2 - dermatology
3 - hematology
4 - rheumatology
5 - neurology
6 - pathology
7 - radiology
8 - med oncology

Dermatological goodbyes and straddling tables..

At long last, I'm finally rid of Derma!.. (I hope)

We just had our Derma final examinations today. Whew! For a one quarter subject, it can sure take out a lot from you.. The test itself wasn't really that hard. I'd say the difficulty would be at par with the sample examinations that were going around.. At least they're consistent. Boom ran into a bit of bad luck today, coming in too late for the exam. But he'll be able to take it with the other section next quarter naman, yung lang nga, there's the hassle of studying again for something you've already prepared for a quarter ago..

During one of our classess, I realy couldn't help but notice one professor who had the habit of straddling the table when he's in the midle of the lecture. Sinasampay niya yung isang leg nya over the side of the table. Think of it like half-sitting on the edge of a table, but in reverse.. hehehe I don't think there's any malice involved, and we respect that teacher a lot.. It's just that we find it pretty amusing.. hahaha =P

Was supposed to have another exam today in surgery.. an exam which I didn't prepare for.. But as my good luck would have it (which has tided me over for the past week or so), the exam was moved and spending my time chilling out at the LRU in lieu of studying became a good move..

I feel so tired lately.. Sobrang hirap gumising in the mornings. I guess it's a combination of lack of sleep and the cold weather. And I guess there's still a little bit of that Christmas vacation hangover. =D
